GUARDIAN QUALIFICATIONS FOR ASSISTANCEIn order to be considered for funding assistance, the Guardian must live in California (except for Kobi's Fund application which is nationwide) and meet one or more of the following qualifying conditions, with documentation: 1.) Be a recipient of state MEDICAID; or 2.) Be a recipient of MEDICARE ; or 3.) Be a recipient of a welfare or other public assistance program ; or 4.) Be a recipient of some form of Disability benefits; or 5.) Be recipient of Unemployment Benefits. Please provide some form of documentation to substantiate your status with any of the above. If you do not qualify under any of the above, please provide information and documentation that you believe qualifies you for assistance. |
